I need to share something important with you: we’re making a significant change to how Cornerstone Health Community operates – one that will give you better tools, more time with our team, and a deeper partnership in achieving your health goals.
What’s Changing (and Why It Matters to You)
Starting January 1st, 2026, we’re moving to a membership + insurance billing model.
Why? Because the current medical industry is failing you; it’s failing all of us.
The average primary care visit in America is now under 18 minutes. Insurance reimbursements keep shrinking. And the care you deserve – root cause analysis, personalized protocols, continuity of care, proactive health planning – isn’t something the traditional model supports or pays for.
We had a choice: cut corners and join the conveyor belt or build something better.
We chose you.
How Our New Model Works
Think of it in two parts:
- Insurance-covered care: Office visits, insurance-approved tests, and diagnostic procedures are billed to your insurance. You pay your copays, deductibles, and coinsurance as usual.
- Membership benefits: The annual membership fee sustainably supports the functional and longevity medicine philosophy & services you value – the deeper dives, the root cause work, the personalized protocols, and the time we spend with you that most insurance companies simply won’t reimburse.
Here’s What You Will Get With the New Model
Annual Comprehensive Longevity Physical (LP) – an advanced assessment that, typically, is reserved for executives and professional athletes.
We’ve spent two years building this, and it’s now yours as a Cornerstone member.
This isn’t a standard annual physical. Here is what this forward-thinking health audit includes:
Longevity Physical Advanced Screening & Diagnostics:
- Preventative imaging: carotid, aortic, thyroid, and renal ultrasound**
- Heart and vascular health: EKG, ABI (ankle-brachial index)
- Brain health: EEG (electroencephalogram) and cognitive function testing
- Neurologic assessment: TM Flow
- Fitness and performance: VO2 Max for heart lung function, body composition analysis, balance testing to assess fall risk**
- Respiratory health: Spirometry test of lung function
- Allergy testing: regional, food, and pet allergens
- Advanced lab panels and biomarkers (similar to Function Health)
** Included in your membership fee. The remaining items are billed to insurance.
Strategic Health Planning Customized for You: After your annual Comprehensive Longevity Physical, we build a personalized strategic health plan that may include:
- Functional nutrition and lifestyle coaching, in collaboration with our Functional Nutrition therapist (included in membership fee)
- Recommendations for genetic testing (MTHFR, COMT, APOE) and Functional Medicine lab testing
- Data tracking tools for sleep quality and stress management tools (WHOOP wearables)
- Toxin elimination strategies and IV infusion therapies
- Weight management and muscle-centric solutions
- Emotional well-being and vagal tone resources

Two Membership Tiers
Peak Plan – $3,500/year – Ideal for those focused on prevention and who are in active pursuit of higher levels of vitality and health span. You’ll work with our Physician Associates and Functional Nutritional Therapist under Dr. Mote’s guidance and protocols. You will receive a customized strategic health plan based on your Comprehensive Longevity Physical results and your health goals.
Transformation Plan – $7,000/year – Ideal for those with complex health issues who need more intensive diagnostic testing and coordination of care. You will work directly with Dr. Mote and our Functional Nutritional Therapist. You will receive a transformational health plan based on your Comprehensive Longevity Physical results and your health goals. These transformational plans typically incorporate multiple functional lab tests and sequenced protocols/treatments to bring about successful health transformation.

Your plan will combine some insurance-covered services, membership benefits, and cash-pay options. However, we’ll be up front about every cost and time investment along the way. Each decision is ultimately yours.

Why it matters: Most standard physicals test 15-20 basic markers. We’re looking at 100+ data points to catch what others miss.

Understanding the Blended Model: Insurance + Membership
We know “membership model” raises questions about cost and insurance. Here’s exactly how it works.
What Your Insurance Covers (Same as Always):

You pay: Your normal copays, deductibles, and coinsurance as determined by your insurance plan.
We remain in-network with most major insurance carriers and accept Medicare.
What Your Membership Covers (New Benefits):
✓ Annual Comprehensive Longevity Physical (a $5,000+ value – most patients will have a small cost share dictated by their insurance plan)
✓ Strategic Health Plan development with extended consultation sessions
✓ Functional nutritional therapy and coaching
✓ Extended appointment times (40-60 minutes)
✓ Priority scheduling with same-week availability
✓ Member discounts on supplements, WHOOP wearables, IV infusion therapies

Can I Use HSA/FSA Funds?
Many patients can use HSA funds to cover membership fees. This varies by plan and there are limitations. To find out definitively, contact your HSA provider. FSA funds can be used for some items, but not for membership fees.

About The Change
Why is CHC changing to a membership + insurance model?
The practice of functional and longevity medicine requires time, personalization, and a proactive approach that traditional insurance models don’t reimburse or fully support.
Our membership fee model allows us to offer deeper, more comprehensive care – including root-cause analysis, lifestyle coaching, and longevity planning – without being limited by insurance constraints.
We are also committed to serving our patients as an in-network primary care provider. As such, we can write prescriptions and facilitate in-network referral services for continuity and ease of care.
Retaining in-network status also allows our patients a direct path for traditional primary care services to be billed to insurance. Note that patients are responsible to pay their insurance-determined cost shares such as co-pays, high deductibles, etc. in addition to the membership fee, strategic health planning, and nutritional therapy. The difference is intensity, testing depth, and who’s your primary provider.

What exactly is a "membership" practice?
Cornerstone’s membership programs offer enhanced services for an annual membership fee. These services go beyond our primary-care insurance-based services and are outside the scope of insurance-covered care.
Our blended model allows us to sustainably offer premier longevity and functional medicine solutions, nutritional therapy, extended appointment times, and priority scheduling while also serving as your primary care provider.
When does this change take effect?
New members longevity physicals start January 1st, 2026.

What's the difference between Peak and Transformation plans?
Peak Plan ($3,500/year) is designed for patients who are generally healthy and want to stay that way—preventing problems before they start. You’ll work with our Physician Associates under Dr. Mote’s guidance.
Transformation Plan ($7,000/year) is designed for patients dealing with complex, chronic health issues who need deeper investigation and Dr. Mote’s direct involvement.
Both include the Comprehensive Longevity Physical, strategic health planning, and nutritional therapy. The difference is intensity, testing depth, and who’s your primary provider.

Family discounts are available. Children aged 22 and under are included in an adult membership (some exclusions apply if children require functional testing).
Can I pay monthly instead of annually?
Membership fees are paid annually upon signing. For families, we offer discounted pricing – call Jennifer to discuss.
You may make two equal installments across two months to cover your annual fee.
You may be able to use HSA funds (check with your plan administrator).
Can I switch between Peak and Transformation plans later?
Yes. If your health situation changes, you can adjust your membership tier at your renewal date.
If you need to upgrade mid-year (for example, a new diagnosis requiring deeper care), talk to Jennifer about changing your plan model.
Insurance & Billing
Does Cornerstone accept my insurance?
Cornerstone is enrolled in most insurance plans and accepts Medicare.
Some plans require that your PCP be part of a hospital-owned group (we’re a private practice), so call your insurance company’s membership services line to verify that we are in-network.
What if I need to see a specialist?
As your primary care provider, we can refer you to specialists within your insurance network.
My insurance already provides an annual wellness physical. Why do I need the Comprehensive Longevity Assessment?
Insurance-covered annual wellness visits typically include:
– Basic vital signs
– 15-20 standard lab markers
– Brief health history review
– Generic preventive counseling
Our Comprehensive Longevity Physical includes:
– 100+ biomarkers (not 15-20)
– Advanced imaging (carotid, aortic, thyroid, renal)
– Neurological assessment (EEG, TMFlow)
– Performance testing (VO2 Max, body composition)
– Respiratory function (spirometry)
– Allergy testing
– Strategic health plan development
– Ongoing coaching and follow-up
The difference: Insurance physicals check if you’re sick. Our Longevity Physical finds problems before you’re sick and builds a plan to keep you that way.
Will you still bill my insurance?
You’re responsible for copays, deductibles, coinsurance, and any amount not paid by insurance (patient responsibility payments).
The membership fee is separate and covers services insurance doesn’t reimburse.
Will insurance reimburse the membership fee?
No. The annual membership fee pays for personalized care and preventive services not typically covered by insurance. This fee is not reimbursable.
However, many patients can use HSA funds to cover membership fees. Check with your HSA provider for specifics.
Can I use HSA/FSA funds for membership?
Many patients can use HSA funds to cover some or all of their membership fees. This varies by plan and there are limitations. FSA plans typically are more restrictive and do not allow for membership fee reimbursement.
To find out definitively, contact your HSA/FSA provider and ask: “Can I use my HSA/FSA for a medical practice membership fee that includes preventive health assessments and health coaching?”

How is this different from regular primary care?
Regular Primary Care:
- 18-minute average appointment
- 15-20 basic lab markers annually
- Reactive (treat symptoms as they appear)
- Provider is rushed, limited time for questions
- Generalized health advice
Cornerstone Membership Model:
- 40-60 minute appointments
- 100+ biomarkers + advanced screening annually
- Proactive (find and prevent problems)
- Extended time for deep discussion and questions
- Personalized protocols based on YOUR data
We’re still your primary care provider for insurance purposes. But we operate at a deeper, more comprehensive level.
What does "functional medicine" mean?
Functional medicine looks for root causes, not just symptom management.
Example: You’re exhausted all the time.
Conventional approach: “You’re stressed. Try to sleep more. Here’s a prescription for anxiety.”
Functional medicine approach: Run comprehensive testing to find out WHY you’re exhausted. Is it thyroid dysfunction? Nutrient deficiencies? Chronic inflammation? Hormone imbalance? Sleep apnea? Gut dysbiosis?
Then address the root cause with targeted interventions, not just band-aid solutions.
For Current Patients
Do I really have to join?
Yes.
We are moving to a membership-based model, and if you’d like us to continue to serve you, you must become a member.
Membership may not be the right answer for everyone. After March 31, 2026, we’ll help transition your medical records to another provider if you’ve decided not to join.
We’ll make sure you have everything you need – medical records, prescriptions, specialist referrals—for a smooth handoff.
We want what’s best for your health, even if that’s not us.
Will my care change if I join?
Yes – it gets better.
You’ll receive the Comprehensive Longevity Physical, strategic health planning, extended appointment times, priority scheduling, and nutritional therapy coaching.
The relationship you have with your provider stays the same (or improves). The depth of care and time available for you increases significantly and we will still be billing insurance for office visits as your in-network provider.
What if I can't afford the membership fee right now?
We understand this is a significant investment. Some options:
HSA: Many members can use these pre-tax funds (check your plan)
Family Pricing: If multiple family members join, the per-person cost decreases
Talk to Jennifer: Explain your situation. She may have suggestions: Schedule a Call
If membership truly isn’t feasible, we’ll support your transition to another provider.
I'm healthy now. Why would I need this?
This is exactly who Peak Plan is designed for.
Most of our members who discovered significant issues felt “fine” before comprehensive testing.
Heart disease, cancer, neurological decline, autoimmune conditions—they all start years before symptoms appear. By the time you feel bad, you’re playing catch-up.
The Longevity Physical catches problems in the “intervention window” when they’re easiest to reverse.
Scenario: Your carotid ultrasound shows early plaque buildup. You have zero symptoms. But now we know and can implement targeted interventions to prevent a stroke 10 years from now.
That’s the entire point of longevity medicine: staying healthy, not getting healthy.
